Our services are uniquely designed to meet USCIS requirements
During the medical exam, The Civil Surgeon will complete a comprehensive examination that will include a review of your medical history and a physical examination (basic checkup).
You will also be tested for communicable diseases such as tuberculosis, syphilis, and gonorrhea, as required by the Centers for Disease Control and Prevention (CDC). Which test are required is based on your age.
The Civil Surgeon will review your vaccination history and go over the vaccination requirements to ensure that you have received all the required vaccinations according to the CDC vaccination guidelines. If any vaccinations are missing, Dr. Green can administer them during the examination if you wish.

A valid government issued photo ID, such as your passport
(preferably) or driver’s license

If you have a copy of a previous I693 bring that with you.
(We have the required I-693 forms in our office, so it is not necessary to bring a copy of a new form.)

VACCINES/IMMUNUIZATIONS
Anyone who seeks to adjust status to that of a lawful permanent resident while in the United States, is required to receive vaccinations to prevent the following diseases

Specific vaccines are based on a clients age
Please bring copies of any immunizations you have received and the provider will review your records and inform you if additional vaccines are required by USCIS. We can administer them during the examination if you wish.
The cost of vaccines and any other questions can be discussed with the provider during your visit.
REQUIRED LAB TEST FOR SPECIFC COMMUNICABLE DISEASES BASED ON AGE
Everyone over 2 years old must have a QuantiFERON Gold TB test
Ages 18-24 years must have a QuantiFERON Gold TB test + Syphilis test + Gonorrhea test
Ages 18-44 years must have a Syphilis test + QuantiFERON Gold TB test
MEDICAL HISTORY REVIEW AND MEDICAL EXAM
The provider will review your medical history, previous illnesses, surgeries and chronic conditions.
Physical examination is a basic checkup of your overall general health which includes measurements of your weight, blood pressure and pulse.
FEES AND PAYMENTS FOR SERVICES
You will be charged for the required medical examination, required laboratory (blood and/ or urine) test(s) and any vaccines you receive.
Acceptable payment options include Cash, Credit or Debit Card.
COMPLETION OF I-693
In 7 business days after your first visit, your paperwork is typically ready for you to sign and receive. The Civil Surgeon and applicant will both sign the original I-693. It is then sealed in an envelope for you to submit to USCIS or your attorney. This envelope CANNOT be tampered with. You will also receive a complimentary copy for your personal records.

What is a Civil Surgeon?
A USCIS Civil Surgeon is a physician designated by the U.S. Citizenship and Immigration Services (USCIS) to conduct medical examinations required for immigrants seeking to adjust their status to permanent residency (i.e., obtain a Green Card) in the United States. These medical exams ensure that applicants do not have any health conditions that would make them inadmissible to the U.S., such as communicable diseases, mental health issues posing a threat, or substance abuse problems. To become a USCIS-designated Civil Surgeon, a physician must meet the following criteria:

Be a licensed medical doctor (M.D.) or doctor of osteopathy (D.O.) in the state where the exam will take place.

Have at least four years of professional experience in their field (beyond training and residency).

Must be approved by the Department of Homeland Security.
What does a Civil Surgeon do?
Civil Surgeons are responsible for performing the medical examination, completing Form I-693 (Report of Medical Examination and Vaccination Record), and ensuring the applicant meets vaccination requirements and is free of communicable diseases. The results are given to the applicant in a sealed envelope. It is submitted by the applicant to USCIS or their attorney as a part of the immigration application process.
What is the I-693?
This form is required if you are applying to become a legal permanent resident of the U.S. The purpose of this form is to confirm that you are not inadmissible to the United States on public health reasons.
How long is the I-693 valid for?
The medical examination is now valid for two years from the date it is signed by the Civil Surgeon. However, the vaccination portion of the exam is typically valid for a lifetime unless USCIS updates its vaccination requirements or if additional vaccines are needed.
Can I get my green card without the I-693?
In order to get an adjustment of status to permanent resident, also known as a green card, an immigrant to the United States is required to meet certain medical standards. The I-693 is used to make sure that an applicant does not have a communicable disease that is a significant risk to public health, has the required vaccinations, is not a drug addict or drug abuser and does not have a physical or mental disorder involving behavior that may be threatening to the safety of the applicant and others.
Can my personal doctor complete my I-693?
No, your personal doctor cannot complete the medical exam for immigration purposes. The examination must be done by a Doctor who is designated by U.S. Citizenship and Immigration Services (USCIS). USCIS designates certain Doctors (known as Civil Surgeons) to perform the immigration medical examination required for green card/adjustment of status applicants.
Will I get a copy of the I-693
The original signed and completed form will be put in a sealed envelope for you to submit to USCIS. You will receive a copy of the completed I-693 for your personal records.
Can I open the sealed envelope?
It is important that you DO NOT open or damage this envelope in any way, as you must submit Form I-693 in a sealed envelope as given to you by the Civil Surgeon to ensure it's authenticity. USCIS will only accept an envelope that is sealed, unopened and not tampered with in anyway.
Can I get a waiver for vaccinations?
It is possible in some cases to be granted a waiver for the vaccine
requirements based on religious or moral beliefs, but this is a separate application process. If you want to apply for such a waiver, you should check with USCIS to obtain a waiver application.
